• Derealization – the present-moment experience that one's surroundings feel unreal, dreamlike, distant, or altered in appearance (e.g., looking through a fog, objects appearing a different size than usual).

• Depersonalization – the present-moment experience of feeling disconnected from one's own body, thoughts, feelings, or sense of self, including watching oneself from a distance or feeling that one's body or being is unreal.

• Identity Confusion – present-moment uncertainty or confusion about who one really is, including feeling like a stranger to oneself or not feeling like a whole person.

• Identity Alteration – the present-moment experience of internal division, competing identities, or a sense that another part or person is present, taking control, or about to take over.

• Conversion – present-moment dissociative symptoms affecting motor, sensory, or speech function without an organic cause, such as weakness, paralysis, abnormal skin sensation, faintness, or seizure-like feelings.

• Amnesia – present-moment difficulty taking in or retaining information, including forgetting what one wants to do or say, or uncertainty about recent events, time, or how one arrived somewhere.

• Hypermnesia – present-moment intrusive or vivid remembering, including flashbacks, unwanted memories, sensory memories (smell, taste, sound), and feelings of deja vu or premonition.

Total Score – overall intensity of dissociative state symptoms across all seven domains at the time of completion.
